Athletic Forms

In order for students to try out for a volleyball team they must have the Acknowledgement of Risk form filled out.

Please fill out the appropriate form for your child (If you fill out the wrong one, coaches will not have your child's name on the list and they will not be allowed to try out).

Grade 7 Boys Volleyball

Grade 7 Girls Volleyball

Grade 8 Boys Volleyball

Grade 8 Girls Volleyball

U13 (Grade 7)

• Must be registered in grade 7 or 8
• Cannot turn 14 before Sep 1, 2024
• Students turning 14 before Sep 1, 2024 will play in U14.

U14 (Grade 8)

• Must be registered in grade 7, or 8
• Grade 8 students who fit in the U13 age group can choose either U13 or U14 (they cannot try out for both)
• Cannot turn 15 before Sep 1, 2024
• Students turning 15 before Sep 1, 2024 will have to participate in U15 with CHS.
